John Smith is a mysterious stranger who is drawn into a vicious war between two Prohibition-era gangs. For the past four years, San Francisco cop Jack Cates has been after an unidentified drug kingpin who calls himself the Ice Man. Jack finds a picture that proves that the Ice Man has put a price on the head of Reggie Hammond, who is scheduled to be released from prison on the next day.

A petty criminal fakes insanity to serve his sentence in a mental ward rather than prison. He soon finds himself as a leader to the other patients—and an enemy to the cruel, domineering nurse who runs the ward.
